At the National Youth Advocate Program (NYAP), we believe in every person’s ability to grow, recover, and thrive. Our Assertive Community Treatment (ACT) services deliver comprehensive, strengths-based support for individuals living with significant mental health and substance abuse challenges—especially those who experience frequent crises or hospitalizations, and who haven’t benefited from traditional outpatient care.
HOW WE SUPPORT YOU
You don’t have to navigate recovery by yourself. NYAP’s ACT program provides individualized support in the places where life happens, at home, in the community, and wherever support is needed most.
Our team helps with mental health treatment, medication support, housing stability, benefits, employment goals, community connections, and everyday challenges so individuals can focus on living healthier, more independent lives.
INDIVIDUALS WHO CAN BENEFIT FROM ACT SERVICES
ACT is appropriate for adults experiencing significant mental health challenges who would benefit from intensive, community-held support.
ACT can be especially effective for individuals with frequent hospitalizations, repeated crisis service utilization, difficulty engaging in traditional outpatient care, housing instability, or challenges maintaining independence in the community

Rather than expecting individuals to navigate care on their own, our team provides coordinated support with mental health treatment, medication management, housing stability, benefits navigation, employment goals, crisis intervention, and daily living skills, meeting people where they are and helping them achieve lasting success.
