Service Components

Assertive Community Treatment (ACT) is a person-centered, flexible service model that provides intensive, community-held support tailored to each individual's clinical needs. 

Services are delivered by a transdisciplinary team of behavioral health professionals who work collaboratively to provide comprehensive, integrated care. Core team members include a psychiatrist, nurse, case manager, clinical supervisor, and peer support specialist, with additional specialized professionals involved as needed to support each individual's recovery goals including:

  • Mental health treatment - Psychiatric evaluation, therapy, symptom management, and coping skill development

  • Physical health coordination - Medication management, health monitoring, and coordination with medical providers

  • Crisis services - 24/7 crisis intervention and support

  • Substance use recovery support - Integrated treatment and recovery-focused services

  • Daily living skills development - Development of ADLs and IADLs, including budgeting, medication adherence, transportation, and independent living skills

  • Employment and educational support - Assistance accessing employment, vocational, and educational opportunities

  • Peer support - Recovery coaching, self-advocacy, and support from individuals with lived experience

  • Individualized goal achievement - Support in pursuing personal goals related to housing, relationships, wellness, recovery, and community participation

Program Goals & Outcomes

ACT is designed to:

  • Reduce psychiatric hospitalizations and crisis service utilization 

  • Decrease frequency and severity of symptoms and crises 

  • Improve housing stability  

  • Support medication adherence and treatment engagement 

  • Increase engagement in recovery-oriented services 

  • Strengthen daily living skills and functional independence 

  • Support personalized, recovery-oriented goals 

Outcomes are individualized and defined in partnership with each person served, with an emphasis on functional improvement, stability, and quality of life.
