ILLINOIS ASSERTIVE COMMUNITY TREATMENT

ACT ELIGIBILITY

ACT services are available to individuals who have significant mental health needs and may benefit from a higher level of support. 

Eligibility includes:

  • Age 18 or older 

  • Serious Mental Illness (SMI) - including schizophrenia, schizoaffective disorder, bipolar disorder, or other severe and persistent mental health conditions

  • Significant challenges with daily functioning 

  • Difficulty engaging in traditional outpatient services 

  • A need for intensive, community-held support 

WHO WE SERVE

ACT services are intended for individuals who require frequent contact, outreach, coordination, and other support to reduce psychiatric hospitalizations, homelessness, crisis service utilization, and additional adverse outcomes. This may include: 

  • Frequent psychiatric crises, emergency department visits, or hospitalizations 

  • Difficulty managing mental health symptoms or medications 

  • Challenges engaging in traditional outpatient treatment

  • Mental health and substance use concerns that impact daily life 

  • Complex physical health needs that affect mental health care 

  • Housing instability or risk of homelessness 

  • Recent suicidal thoughts, self-harm, or other significant safety concerns 

  • Difficulty maintaining relationships, employment, or daily responsibilities because of mental health symptoms 

  • A need for intensive, coordinated services to reduce hospitalization and promote community living

ACT GOALS

ACT is designed to support greater stability, reduce mental health crises and hospitalizations, and help you manage symptoms in ways that work for your daily life. We also support the development of functional life skills while working toward safe, stable housing whenever possible.

Most importantly, ACT helps you move toward your own goals, whether that’s increasing independence, improving quality of life, or maintaining meaningful connections to your community.
