Physical health coordination – Assistance with staying healthy, monitoring health needs, and connecting with medical providers
Crisis support - 24/7 support during times of crisis or when extra help is needed
Substance use recovery support - Recovery services and support for individuals working toward wellness and recovery
Daily living skills development - Support with everyday skills like budgeting, transportation, taking medications, and living independently
Employment and educational support - Help finding job opportunities, career support, and educational resources
Peer support - Encouragement, guidance, and support from people with lived experience
Personal goals and life planning - Support in reaching personal goals related to housing, relationships, health, recovery, and community involvement
