24/7 crisis support when you need immediate help

Support with independent living skills, including performing household routines, self-care, and maintaining a safe living environment
Connections to essential community resources—including housing, transportation, employment, and healthcare—to strengthen your long-term wellness and independence
Ongoing outreach and engagement to support consistency of care
Who We Serve
ACT is designed for individuals aged 18 and older with complex mental health and substance abuse challenges that greatly impact their ability to live independently within their community. This may include people who are:
Experiencing frequent mental health crises or hospitalizations
Finding that traditional outpatient services have not worked well for them
Experiencing homelessness or housing instability
Impacted by justice system involvement or at a higher risk of involvement
ACT Goals
ACT is designed to support greater stability, reduce mental health crises and hospitalizations, and help you manage symptoms in ways that work for your daily life. We also support the development of functional life skills while working toward safe, stable housing whenever possible.
Most importantly, ACT helps you move toward your own goals, whether that’s increasing independence, improving quality of life, or maintaining meaningful connections to your community.
