Front Door Diversion Program (FDDP)

At the National Youth Advocate Program (NYAP), we believe everyone deserves support that reflects their strengths, needs, and goals. The Front Door Diversion Program (FDDP) offers an alternative path for adults with mental health needs as they prepare to leave hospital-based care. 

We work alongside each individual to explore community-held options that support greater independence, stability, and overall well-being. 

What We Offer

FDDP provides person-centered, strengths-based services to help individuals transition safely from higher levels of care into less restrictive settings: 

  • Individualized clinical support from licensed professionals

  • Help meeting immediate needs and finding stable housing

  • Coordination of outpatient care, medication management, and other needed medical services

  • Connections to community resources that support daily living and skill development

  • Additional supports such as employment services, peer support, and substance abuse prevention, as needed 

  • Support in building meaningful social and natural support networks  

Who We Serve

The Front Door Diversion Program supports individuals who: 

  • Are Medicaid-eligible adults with mental health needs 

  • Are currently admitted to a psychiatric unit within a partner hospital  

  • Are being considered for discharge to a Specialized Mental Health Rehabilitation Facility (SMHRF)  

  • Have completed a Pre-Admission Screening  

  • Have been assessed by an FDDP team member and choose to participate in services

We focus on helping individuals move forward with confidence and the right supports for lasting stability within the community. Through FDDP, we aim to: 

  • Support safe and successful transitions from hospital care to community living 

  • Promote independence, stability, and an improved quality of life  

  • Reduce reliance on long-term, restrictive care settings  

  • Keep individuals connected to the services and supports they need

FDDP connects with individuals early in the discharge process to explore options and offer meaningful alternatives to institutional care. Our approach is centered on choice, respect, and practical support—meeting individuals where they are and helping them build a path forward within their communities.
