At the National Youth Advocate Program (NYAP), we believe every person deserves an opportunity to achieve stability and independence. Through our Comprehensive Class Member Transition Program (CCMTP), we support individuals transitioning from Specialized Mental Health Rehabilitation Facilities (SMHRF) to independent living situations within their communities.
What We Offer
Our services are strengths-based, person-centered, and designed to promote lasting independence, including:
Individualized clinical services provided by licensed professionals
Assistance in securing safe, long-term housing
Counseling, case management, and rehabilitation
Access to programs that build daily living, social, and developmental skills
Specialized services for supported employment, peer support, and substance use prevention, as appropriate
Access to registered nursing and psychiatry services for medication management and monitoring
Who CCMTP Serves
Illinois residents, age 18 and older, who:
Have a mental health condition
Are currently residing in Sharon Woods Healthcare
May be able to live successfully in the community with proper supports and services
Are willing to complete the assessments provided through the partnership between NYAP and the Illinois Department of Human Services, Division of Mental Health
Our Goals
The Comprehensive Class Member Transition Program helps individuals move toward greater independence while maintaining stability in the least restrictive setting possible. Our goals include:
Supporting successful transitions to community living
Enhancing independent living skills
Preventing destabilization of mental and behavioral health conditions and recidivism through consistent, compassionate support
