Care Coordination
NYAP is an experienced Care Management Entity (CME) that works with community partners such as service providers, public children's services agencies, and other stakeholders to develop, enhance, and expand the local system of care. Through care planning, NYAP coordinates supports and services for individuals with moderate and high behavioral health needs and their families, helping them navigate a complex and often confusing multi-system environment.
